HFT Sheffield and Derbyshire

Individual people, individual choices, individual lives.

Lets be straight, we do not provide support to people in a large care Home and we are not a working Farm.  We are however someone you can Trust.

HFT Sheffield/Derbyshire is able to provide a variety of diverse, flexible and creative support packages to adults with a learning disability in any environment, at any time of day, for any period of time.

We believe individual service design is paramount and through ongoing person centred planning support packages can be tailored to support a person to have a clear path towards greater independence and to live their life, their way.

Support staff are employed and trained based on job descriptions and person specifications drawn up by the person they will be supporting.

We are at the forefront of modern support using assistive technology to enable people to be as independent as possible.

There is access to a supported employment worker who provides hands on support and advice through skill matching, job carving and maximising peoples potential to enable them to find meaningful employment in the community.

We run a stand alone community inclusion service which supports people to access a wide range of activities and facilities in Sheffield and Derbyshire.

We work in partnership with both Golden Lane and South Yorkshire housing associations and can support people to look at other housing options such as shared home ownership.

People we support are directly involved in how HFT runs both now and in the future.

So whatever lifestyle people want to live, whatever goals people want to achieve, we can provide the support they may need.
