Supporting People with Learning Disabilities

HFT Cheshire and Wirral

HFT Cheshire and Wirral is able to provide a variety of diverse, flexible and creative support packages to adults with a learning disability in any environment, at any time of day, for any period of time. Individuals are able to access our services using a range of funding options, including direct payments, Resource allocations, and privately funded packages.

We believe individual service design is paramount and through ongoing person centered planning support packages can be tailored to support a person to have a clear path towards greater independence and to live their life, their way. Support staff are employed and trained based on job descriptions and person specifications drawn up by the person they will be supporting. We are at the forefront of modern support using assistive technology to enable people to be as independent as possible.

We currently provide support to a number of people living in their own homes, in supported tenancies and in residential care services. We operate a stand alone community inclusion service, there are a number of sessions that we run in individuals own communities. Individuals can access this service as part of a larger care package or drop in to specific activities; this is a popular option for individuals in receipt of a direct payment. There is a directory of community inclusion services available upon request. Our supported employment worker can also provide support and advice around employment.

We have experience of working with various housing associations and can support people to look at many different housing options. People we support are encouraged to become involved in how the service is run, and views are sought on a regular basis. A service user guide/statement of purpose, community inclusion guide and various inspection reports are available upon request. 

So whatever lifestyle people want to live, whatever goals people want to achieve, we will work towards providing the support they may need.
