Hft is a charity providing services for learning disabled people throughout England and Wales. We provide support from just a couple of hours a week up to 24 hours a day. Our staff teams support you to live your best life possible.

HFT Fusion Model

We put people at the centre of everything

Our Fusion Model is based on Person-Centred Active Support (PCAS) – a way of supporting people with learning disabilities so they are engaged in meaningful activity and relationships as active participants. As a result, they exercise more control over their lives and experience greater levels of inclusion, independence, and choice. Our Services

From supported living to residential care, day opportunities to transition services, Hft staff help the people we support to live with more choice, more independence and with greater access to the community.

  • With supported living, we support people to live however and wherever they choose. We can help with finding the right accommodation and with ongoing support

  • We provide domiciliary care to people who require support with activities they would find difficult on their own, to help them live with more independence.

  • Our Day Opportunities centres allow the people we support to take part in things they enjoy and make choices about when they do them.

  • Hft offers residential care run to high standards, with a focus on supporting people to achieve the best possible outcomes.

  • Our flexible short breaks and respite care services help allow people with learning disabilities and their families or carers to enjoy some 'me time'.
