Our Work

Our person-centred approach supports people to take control of their lives. We work closely with the people we support, their families and carers, and health professionals to deliver services that are personalised to individual needs. Our support workers empower the people we support to make their own choices about how they live, using our innovative Fusion model of support to keep people at the centre of everything we do.

At Hft London we support people with many different types of learning disability, and have particular experience in supporting people with Down’s Syndrome and dementia, autism, and challenging behaviour. Staff are also trained in positive behavioural support and management, person-centred active support, epilepsy management with emergency medication, dysphagia, dementia and end of life care.

Our emphasis is on creatively supporting people to live with greater levels of independence and personal fulfilment, to achieve the best possible outcomes.
