Our person-centred approach supports people to take control of their lives. We work closely with the people we support, their families and carers, and health professionals to deliver services that are personalised to individual needs.
Our support workers empower the people we support to make their own choices about how they live, using the social model of disability to keep people at the centre of everything we do.
At Hft Cheshire and Merseyside we support people with many different types of learning disability, and have particular experience in supporting people with autism spectrum disorder who may also have complex or challenging behaviours, and physical or mental health conditions. We also provide a specialist service for people with Prader Willi syndrome. What’s consistent throughout, is our focus on ensuring that everyone is supported to live the best life, and achieve the best outcomes possible.

We support learning disabled people to live however and wherever they choose. We can then provide support in a way that suits each person.
