Our person-centred approach supports people to take control of their lives. We work closely with the people we support, their families and carers, and health professionals to deliver services that are personalised to individual needs. Our support workers empower the people we support to make their own choices about how they live, using our innovative Fusion model of support to keep people at the centre of everything we do.
At Hft Cornwall we support people with many different types of learning disability, and have particular experience in supporting people with Prader Willi Syndrome and challenging behaviour. What’s consistent throughout though, is our focus on ensuring that everyone is supported to live the best life, and achieve the best outcomes possible.

With supported living, we support people to live however and wherever they choose. We can help with finding the right accommodation and with ongoing support
We provide domiciliary care to people who require support with activities they would find difficult on their own, to help them live with more independence.
Our Day Opportunities centres allow the people we support to take part in things they enjoy and make choices about when they do them.
Hft offers residential care run to high standards, with a focus on supporting people to achieve the best possible outcomes.
Our flexible short breaks and respite care services help allow people with learning disabilities and their families or carers to enjoy some 'me time'.
