Our person-centred approach supports people to take control of their lives. We work closely with the people we support, their families and carers, and health professionals to deliver services that are personalised to individual needs. Our support workers empower the people we support to make their own choices about how they live, using our innovative Fusion model of support to keep people at the centre of everything we do.
At Hft Shropshire and Staffordshire, we support a range of individuals from people living independently in the community, to people with profound and multiple learning disabilities who require support 24 hours a day.
At our Day Opportunities centre we have installed a hydrotherapy pool to help ensure the best possible health outcomes for those people we support who benefit from this form of physiotherapy. What’s consistent across all of our services though is our focus on making sure that everyone supported by Hft has the opportunity to live the best life possible.
For more information on the action plan we’ve put in place for Wrekin Cottage, please contact us.
Take your first steps towards a rewarding career that offers you the opportunity to make a real and positive difference to someone’s life every day.
With supported living, we support people to live however and wherever they choose. We can help with finding the right accommodation and with ongoing support
We provide domiciliary care to people who require support with activities they would find difficult on their own, to help them live with more independence.
Hft offers residential care run to high standards, with a focus on supporting people to achieve the best possible outcomes.
Our Day Opportunities centres allow the people we support to take part in things they enjoy and make choices about when they do them.
Our flexible short breaks and respite care services help allow people with learning disabilities and their families or carers to enjoy some 'me time'.
