Supporting people with learning disabilities to live with more independence and choice across Sheffield, Derbyshire, and the surrounding areas for over 50 years.
Our person-centred approach supports people to take control of their lives. We work closely with the people we support, their families and carers, and health professionals to deliver services that are personalised to individual needs. Our support workers empower the people we support to make their own choices about how they live, using our innovative Fusion model of support to keep people at the centre of everything we do.
At Hft Sheffield & Derbyshire we support people with many different types of learning disability in a variety of different settings. We have particular experience in supporting people with autism, complex needs, mental health issues, and challenging behaviour.
Our emphasis is on creatively supporting people to live with greater levels of independence and personal fulfilment. We currently support around 85 people across the region to live the best lives possible.
